You need to get three attributes through your actions in the game. Picking the rose got you one. Try getting the heart first and look at it, then get the rose and look at the heart again. You'll see

In Mala Schizala, when at the crossroads, go up to the altar room. Pick up the box there, and you'll have the heart.

The heart's description and pic changes, depending on which attributes you've already collected. If you picked up the rose before the heart, you won't notice the change, as it'll automatically show the changed version
