This is a fault of the game itself, infact also the Amiga version had this problem (much less). A solution can be the same suggested for 1) by Fifth Horseman and also increase sensitivity in .conf file of DOSBox.

Try various CPU cycles (more or less) and various sensitivity values (always more then 100). If, after that, you are unable to find an optimal setting, simply disable sound effect or music <_<

Honestly, the trauma I've been going through trying to play this game today! I even tried a few Amiga versions on WinUAE, but the only one that would work was in German!!! :'(
