TV show : Mission impossible.
 Movie : Rocky III's "Eye of the tiger".  Best mix-up of both : Watching Baywatch opening credits, without TV sound, listening to the Eye of the tiger.  |

TV Show: I don't know, I don't watch a lot of TV...infact, I never do...although the Simpsons Theme is pure genius (but given by who it was written by, of course it would be)

TV Show: there are lots of good old TV themes. Miami Vice was good, I also like The Simpsons, Buffy, Greatest American Hero, Charmed
Movie: The Godfather, Imperial March (Star Wars), Kill Bill theme

TV Show: Can't think of anything but The Simpsons for now. But just as Reamus said, that one's (at least) genius.
Movie: Gotta say The Godfather. Of course meaning the real Godfather theme (aka. The Immigrant, I think), not Speak Softly Love, which many people think is the theme. Although it is a good composition too, it still can't be compared to The Immigrant.
